After many years of dedication to our patients, and families, in the Fort Worth community, we are relocating our pediatric dental practice.  We thank you for your commitment to our practice, and doctors, to provide pediatric excellence in care for your child/children.  We are ecstatic to be able to continue pediatric care at the following location with Dr Ali Al Aqeeli (Dr. Ali) in Hurst, TX:

Pursuant to Texas law and the rules of the Texas Board of Dental Examiners we are required to provide you no less than 30-day’s notice of that we are relocating the Practice and that we intend to close this location and move to the facility set forth above.  To that end, the letter serves to inform you that we will completely shut down this practice location on or about August 30.  If you have a continuing plan of care and have a fully paid balance with the Practice, we will continue to treat you at our new Practice location.  We strongly recommend maintaining the continuity of your care at our new location.  At the same time, we understand you may wish to obtain care from another dentist and upon written directive from you will transfer your records to any such dentist you elect.  As required by Texas law we will maintain records of all minor patients until their 21^st birthday.
